REFERRAL PROCESS

A clear pathway to understanding what comes next.

Every young person referred to Eden House is considered individually. Our referral process helps us understand the young person’s circumstances, educational needs, strengths and support requirements before a placement pathway is agreed.

STARTING A REFERRAL

Begin with a professional conversation.

Referrals can be discussed with Eden House when a young person may benefit from a personalised Alternative Provision pathway. We encourage schools and professionals to contact us to discuss the young person’s circumstances before a placement is agreed.

01

Who can make contact?

We work with schools, local authorities and relevant professionals to understand whether Eden House may be an appropriate pathway for an individual young person.

01

Schools & academies

Mainstream schools, academies and multi-academy trusts can discuss potential referrals.

02

Local authorities

Local authorities and relevant professionals can also make contact regarding a potential placement.

03

Parents & carers

Parents and carers should normally speak with the young person’s school or relevant professional in the first instance.

04

Individual consideration

Suitability is considered individually and a referral does not automatically result in acceptance.

!
Referral discussion is not automatic acceptance

Eden House considers the information available, the young person’s circumstances and whether the provision can be appropriately supported before a placement pathway is agreed.

THE REFERRAL JOURNEY

Five steps from initial discussion to review.

The process is designed to gather the information needed to understand suitability before a placement is agreed.

01

Referral

Initial contact and discussion about the young person’s circumstances.

02

Information Gathering

Relevant educational, SEND, safeguarding and professional information is gathered.

03

Screening & Suitability

Information is reviewed to understand whether Eden House may be appropriate.

04

Placement Planning

Where appropriate, a placement pathway is planned around the young person’s needs.

05

Review & Next Steps

Progress and support are reviewed and future steps considered.

Suitability is determined through professional discussion and assessment.

The process is not based on a simple checklist. Eden House considers the young person’s individual circumstances, needs, support requirements and whether the proposed placement can be safely and appropriately supported.

STEP 02

Referral & Information Gathering

Eden House gathers the information needed to understand the young person’s current circumstances and educational starting point.

  • Referral information and educational history
  • Academic starting points and current engagement
  • Existing assessments and relevant professional information
  • SEND information and identified support needs
  • Safeguarding and relevant risk information
  • Information from the referring school and professionals
STEP 03

Screening & Suitability

The information provided is reviewed as part of Eden House’s referral screening and suitability process.

  • Individual needs and circumstances
  • Academic starting points and educational barriers
  • SEND, SEMH, behaviour and emotional regulation needs where relevant
  • Safeguarding and risk information
  • Support required to access and engage with education
  • Whether the proposed placement can be appropriately supported
STEP 04 — PLACEMENT PLANNING

Agreeing an appropriate placement pathway.

Where Eden House is considered appropriate, the next step is to agree a placement pathway around the young person’s needs. This includes establishing the purpose of the placement, key priorities, timetable and support.

01 Agreed placement pathway and duration
02 Learning and personal development priorities
03 Timetable and attendance arrangements
04 Support strategies and adjustments
05 Communication and review arrangements

STEP 05 — REVIEW & NEXT STEPS

Reviewing progress and planning what comes next.

Progress is reviewed throughout the placement. Reviews help Eden House and the relevant adults and professionals understand what is working, what progress has been made, what barriers remain and whether the placement or support needs to change.

01 Continue the agreed pathway
02 Extend or progress where appropriate
03 Transition back to the home school
04 Progress to another suitable setting
05 Prepare for post-16 pathways where appropriate
